Full time position:
Monday-Friday - 8:00 am to 4:30 pm or 8:30 am to 5:00 pm
Job Responsibilities/Duties:
• Perform initial and ongoing nursing assessments
• Develop and maintain patient’s individualized plan of care
• Provide education and support to patients and caregivers regarding disease process, self-care techniques, end of life care, comfort measures, medication use and side effects
• Pronounce deaths in accordance with state regulation, provides postmortem care
• Coordinate care by LVNs and CNAs, DME and medicals supplies
• Obtain orders for medications and ensure medications are refilled/changed as appropriate
• Provide supervision of care provided by LVNs and CNAs
Certifications/Licenses:
• Current, unrestricted Texas RN license
• Current driver’s license
• CPR certification
Position Requirements:
• One year nursing experience required; hospice or acute care experience, preferred
• Ability to work independently and as a member of the interdisciplinary team
• Excellent communication and interpersonal skills
• Proficient electronic medical record and computer skills
• Exceptional documentation skills
• Reliable transportation and current automobile liability insurance
