Position available:  Monday – Friday, 8 am to 5 pm (on site)

Job responsibilities:
•    Provides clerical support to Patient Care Manager and interdisciplinary team in the facilitation and coordination of hospice services, including, but not limited to, provide telephone support to patients/families to confirm visits, maintain medical records for team’s patients, prepares for interdisciplinary team meetings, schedule meetings for Patient Care Manager. 
•    Coordinates the delivery of DME for patients between vendor and patient/family.
•    Coordinates the delivery of medical supplies for patients.
•    Coordinates ancillary services for patients.
•    Serves as scribe for interdisciplinary team meetings.
•    Data entry into electronical medical records..
•    General clerical duties.

Position requirements:
•    High school diploma or equivalent
•    Minimum one-year clerical assistant or medical assistant experience in a medical environment
•    Proficiency in Microsoft Office and data entry
•    Electronical medical record system experience
•    Comfortable navigating through medically related vendor web-based programs
•    Ability to work independently and as a member of the interdisciplinary team
•    Excellent phone etiquette
•    Exceptional communication, interpersonal and organizational skills
•    Reliable transportation and current automobile liability insurance
•    Valid Texas driver’s license
